Судя по вашему описанию, в форме «Records survey form – aextjfo» есть поле, которое помечено как обязательное ( `required` ), но в реальном процессе заполнения это поле иногда может оставаться пустым.
При попытке отправить форму сервер отклоняет запрос и выдаёт ошибку валидации – потому что в бекенде тоже прописано, что поле должно быть заполнено.
